Care Connection Home Care LLC services can accommodate your schedule on a short or long term basis, seven days a week or by the hour, day, overnight or on a live-in basis. Specific care plans are designed for each person and their unique lifestyle needs.

Our RNs & LPNs

Our RNs and LPNs are dependable, experienced caregivers. In-home visits begin with a thorough assessment of your loved ones health to determine the care that is right for them. Regular home visits are then scheduled to monitor their wellness. Our RNs and LPNs duties include; a clinical assessment prior to the start of care, initial home visit and safety assessment, creating a customized plan of care, ongoing case management and RN supervision visits, 24 hours a day, 7 days a week.

 

Our HHAs and PCAs

Following a thorough assessment by our nurse and under her supervision, a Certified Home Health Aide or Personal Care Aide may be placed with your loved one. HHAs and PCAs duties include; preparing meals that address dietary or health issues, assisting with bathing, dressing, grooming and toileting, assisting with transfers and ambulation, performing healthcare related tasks such as monitoring vital signs and medication under the direction of our registered nurse.

Our Companions & Homemakers

Often times, a nurse assessment will determine that all a loved one would need to remain safely and securely in their home is the company of a friendly and compassionate companion or homemaker. The nonmedical services of a companion or homemaker include: preparation of meals, light housekeeping and laundry, shopping including groceries and clothing, escorts or driving to medical appointments and social gatherings, medication reminders, grooming and dressing guidance, and assuring safe mobility throughout the client’s home.


WHENEVER YOU NEED US, WE’RE THERE FOR YOU

Care Connection Home Care draws from a wide pool of skilled and experienced home care professionals. Our caregivers come to us through reputable referral sources with training as nurses, home health aides or personal care aides. Only skilled and experienced companions will be placed with a client in need of non-medical home care assistance. Because a large number of our caregivers drive, they can easily get to clients in all areas of Long Island and the NY metro region.

You can feel safe and secure with our home caregivers. They undergo a thorough screening and training process that includes two recent employment references and criminal background checks. We also access current health records. All caregivers are briefed about the health, dietary needs, interests and hobbies of each client so they can encourage the client to heal properly giving the family peace of mind. If a caregiver observes a change in the health status or a medical emergency arises, they follow specific guidelines set by Care Connection Home Care.


TYPICAL HOME CARE STAFF DUTIES

Hospital Care

Even though the client is being treated by hospital staff, many individuals and elderly clients still need the personal attention of a private duty caregiver to oversee their day to day needs.

During a hospital stay, your loved one may become debilitated and incapacitated so they may not be able to feed themselves or contact a nurse’s station when necessary. A privately placed caregiver from our agency can offer that extra measure of protection and advocates on behalf of the client, helping to enlighten and brighten their hospital stay.

Home Care

First and foremost our caregivers form a bond and friendship with your loved one. We want visits from our staff, whether a nurse, personal care aide, home health aide, companion or homemaker, to be something your loved one looks forward to every day. 

While at the home, our staff not only provides the medical and personal care prescribed by a doctor’s care plan, but they also typically provide assistance with meal planning and preparation, shopping, laundry, groceries, social support, transportation and errands, medication reminders, light house-keeping, and driving to appointments.

Assisted living facility

At times, a resident may benefit from the company of a private duty caregiver, thereby enhancing their experience at the facility.

In addition to providing assistance with personal care needs and advice on dressing, the caregiver can engage the client in activities and conversation, accompany them on trips and even provide a friendly and familiar face while watching t.v. or movies. Our caregivers can also retrieve mail, do light shopping and escort the client to the dining room, social events and other facility activities.
